STUDENT INFORMATION

Accessible Learning Services is responsible for the review and approval of student accommodations, guided by Accessibility and Human Rights Legislation. To receive accommodations at Acadia, students complete a three step process, initiated through this form.

  • STEP 1: sUBMIT iNTAKE REQUEST & dOCUMENTATION
  • Step 2: Schedule & Attend intake meeting
  • STEP 3: Receive confirmation of your accommodations

Note: You may begin the intake process if you do not have documentation available. Howeverdocumentation is required to provide students with accommodations in post-secondary. 


We recognize that everyone has access to different resources which may represent a barrier to accessing current documentation; and may be able to accept some forms of documentation to determine eligibility for limited and/or temporary accommodations.

Documentation Requirements

To receive appropriate accommodations and support, students provide Accessible Learning Services with documentation completed by a licensed medical practitioner within the past 5 years identifying their strengths, functional limitations, and recommended supports.

 

This may include:

  • psycho-educational/neuro-psychological assessment (less than 5 years old)
  • psycho-educational/neuro-psychological (5+ years old), accompanied by a letter from psychologist/psychiatrist verifying its accuracy and/or recommendations for post-secondary

If applicable, temporary accommodations will be discussed during the intake meeting.

By submitting this form I certify that the information I have provided is accurate to the best of my knowledge. I understand that misrepresenting or providing false information to Accessible Learning Services for the purposes of receiving an advantage is considered academic dishonesty and may be reviewed under the Academic Integrity Policy.

I also understand that information I provide to Accessible Learning Services is considered confidential, as governed by the Nova Scotia Freedom and Information and Protection of Privacy Act. This means we will not share your information without your explicit consent unless exceptional circumstances occur, such as:

  • There is a concern for your safety or others;
  • The information is part of an academic integrity case;
  • We are legally required to do so (such as a court subpoena).



Furthermore, registering with our services does not appear on student's transcripts and is stored in secure location separate from the rest of your student records.
